Evaluation / Certificate. At the conclusion of the course, upon verification of attendance, course completers will receive an email with a link to the course evaluation. Allow a week for the email to come.
When the evaluation is completed, you will be automatically redirected to your course completion certificate.
Type your name in the space provided, then save the PDF and print it. Keep in a place where you know you can retrieve it (i.e. personal laptop, email to your personal email).
You have 60 days to obtain your course completion certificate by completing the course evaluation. After 60 days, anyone needing a copy of their course completion certificate will be required to pay $30 by credit card to obtain a replacement certificate.
Certificate: The SPICE course completion certificate is the document that verifies that you have completed the infection control course that is required by the State of North Carolina for persons designated to oversee the infection control program in the facility where they work. Your current or future employer may require a copy of this document.
