IP colleagues who are submitting a recertification application and would like to obtain a letter of credit for teaching NC Infection Control Curriculum (Outpatient, Hemodialysis, Home Health/Hospice, Dental), please complete this form.
The form requests your name, email address, and date or dates that you taught the course. A letter will be sent to you via email that you can include with your submission.
